Title: Home Care RN Case Manager
Location: Poughkeepsie, NY
Compensation: $100,000 – $125,000 + large sign-on bonus
Benefits:

Responsibilities for Home Care RN Case Manager:
• Provide in-home care primarily to geriatric patients
• Perform skilled nursing services including wound vacs, PleurX drains, INR draws, dressing changes, and more
• Conduct start-of-care admission visits and re-visits
• Educate and communicate with patients and family members regarding medical conditions and care plans
• Manage a manageable daily caseload of 4–5 patients per day
• Document clinical notes accurately within the EMR system
• Collaborate closely with interdisciplinary home care team members

Preferred Qualifications for Home Care RN Case Manager:
• Associate Degree in Nursing (ADN)
• 1+ year of Registered Nurse experience
• Active New York State RN license
• BLS certification
• Prior home care experience strongly preferred
