We are excited to announce a new opportunity as a Hospice Admissions Nurse for our rapidly expanding organization. Our organization has been around for over 100 years and we focus on treating geriatric patients in rehabilitation and long term care facilities throughout New York City and Long Island. As the Hospice RN Admissions Nurse, you would be working alongside a supportive team and have a multitude of job functions. Title: Hospice Admissions RN
Location: Manhattan NY
Salary: $90,000 to $120,000

Responsibilities:
Completing assessments of patients as assigned
Working closely with care coordinators
Completing safety assessments of patients
Documenting necessary notes in EMR system
Reviewing medications and providing guidance to patients
Communicating with patient family members when needed
Working with Hospice staff and completing clinical reports
Performing in depth assessments of patient’s physical health
Obtaining physician orders during time of admission
Providing documentation for all equipment and supply orders needed for patient
Educating family members on patient care
Reviewing medical history and providing ongoing active treatment

Requirements:
Associates Degree of Nursing
At least 1 year of experience in long term care setting, hospital, hospice, or oncology
Prior Admissions nursing experience a plus
Active New York state RN license
BLS
Strong interpersonal and communication skills
