Our healthcare organization is rapidly expanding our treatment services and we are seeking a Home Care Registered Nurse Case Manager to see patients in the field and perform start of care admission visits and re-visits. You would be a part of a large hospital system and have a fantastic opportunity to expand your nursing skills. Title: Home Care RN Case Manager
Location: Pleasant Valley/ Poughkeepsie
Compensation: $90,000- $120,000 + large sign on bonus
Benefits: Health Insurance, Dental, Vision, Life, Disability, malpractice, CME, 33 days PTO, tuition reimbursement, CME Credit

Responsibilities for Home Care RN Case Manager:
Seeing mostly geriatric patients in their homes and doing wound vacs, plurex drains, INR draws, dressing changes, etc.
Speaking with family members about patient medical ailments
Doing start of care admission visits and re-visits
Seeing 4-5 patients per day
Documenting notes in EMR system
Interacting with other home care staff members

Preferred Qualifications of Home Care RN Case Manager:
Associates Degree of Nursing
1+ Year Registered Nurse experience
Active New York RN license
BLS
Home care experience preferred
