Title: Home Care RN Case Manager (Bilingual Spanish)
Location: Danbury, CT
Salary + Benefits: $95,000- $120,000 plus a comprehensive benefits package including Medical, Dental, Vision, PTO, retirement plan, mileage reimbursement, and opportunities for professional growth

Responsibilities of Home Care RN Case Manager:
• Manage a caseload of patients receiving home health services, ensuring individualized and high-quality care
• Conduct initial assessments, develop care plans, and perform regular reassessments in patients’ homes
• Provide skilled nursing care, including wound care, medication management, and health monitoring
• Educate patients and families on disease management, medications, and post-hospital care
• Coordinate care with physicians, therapists, social workers, and other members of the healthcare team
• Monitor patient progress and adjust care plans as needed to achieve optimal outcomes
• Complete timely and accurate documentation in the EMR system
• Ensure compliance with all regulatory and clinical standards
• Provide culturally competent care, effectively communicating with Spanish-speaking patients and families
• Participate in case conferences and interdisciplinary team meetings

Requirements of Home Care RN Case Manager:
• Active Connecticut Registered Nurse (RN) license
• Bilingual in Spanish (required)
• Minimum of 1 year of RN experience; home care or case management experience preferred
• Strong assessment, organizational, and care coordination skills
• Ability to work independently in a field-based setting
• Excellent communication and patient education skills
• Valid driver’s license and reliable transportation
• Proficiency with electronic medical records (EMR) systems
