As a Home Care Hospice RN Case Manager, you will play a vital role in supporting patients and families during end-of-life care—providing comfort, dignity, and expert clinical coordination. You’ll see patients in their homes, hospitals, or nursing facilities, managing care for approximately 5 patients per day.
This role offers an excellent work-life balance with a 35-hour work week, Monday–Friday, 9:00 AM–5:00 PM. Please see position highlights below.

Title: Home Care Registered Nurse – Hospice Case Manager
Location: Brooklyn or Queens
Salary: $110,000 to $140,000 + Full Comprehensive Benefits Package

Responsibilities:
• Conduct comprehensive assessments of patients’ medical conditions and medication effectiveness
• Accurately document all clinical notes in the EMR system
• Review and implement physician orders related to the patient’s plan of care
• Monitor patient progress and coordinate timely follow-up visits
• Provide ongoing in-field assessments while visiting patients in various settings
• Deliver direct patient care in alignment with the Interdisciplinary Plan of Care, ensuring maximum comfort and quality of life
• Serve as the primary case manager, assuming full responsibility for care coordination
• Perform ongoing evaluations and revise the written plan of care weekly—or as patient/family needs evolve—in collaboration with the interdisciplinary team
• Authorize, coordinate, and supervise care with contracted vendors to meet individualized patient needs
• Maintain consistent, effective communication with all hospice team members involved in patient care
• Participate in patient intakes and information visits as needed, including explanation of hospice and Medicare benefits, full physical assessments, and completion of all required documentation
• Provide bereavement resources and emotional support to families when appropriate
• Assess home care needs with sensitivity to physical, emotional, spiritual, social, cultural, and economic factors
• Communicate with empathy, professionalism, and respect while maintaining confidentiality and patient privacy

Requirements:
• Associate’s Degree in Nursing (BSN preferred)
• Minimum of 1 year of hospice/home care/ or med/surg nursing experience
• Active New York State Registered Nurse license
• Valid driver’s license
• Must speak Mandarin or Cantonese
• Compassionate, patient-focused approach to care
• Home care nursing experience strongly preferred
