Hospice RN Team Manager

We are actively seeking an experienced Hospice RN Team Manager for a full-time, permanent role with a highly regarded healthcare organization serving patients and families across Brooklyn and Manhattan. This is a leadership position that balances hands-on clinical support with mentoring and operational oversight of nursing services in hospice and palliative care settings.

Title: Hospice RN Team Manager
Location: Brooklyn / Manhattan, NY
Salary: $140,000 to $155,000

Responsibilities:
• Support the Associate Director of Patient Services by overseeing and guiding hospice nursing operations on a daily basis
• Provide mentorship and leadership to RN Case Managers with a focus on pain and symptom management and patient/family-centered goals of care
• Collaborate with the Associate Director to schedule staffing assignments in alignment with NHPCO and internal staffing standards
• Ensure quality of care by reviewing and monitoring nursing documentation for compliance with state, federal, and organizational policies
• Work collaboratively within an interdisciplinary team to promote cohesive patient care
• Participate in outreach efforts and deliver community presentations in partnership with admissions and outreach departments

Requirements:
• Graduate of an accredited School of Nursing (BSN or BS preferred)
• Minimum 5 years of hospice nursing experience
• Active RN license in New York State
• Strong ability to mentor, supervise, and support hospice nursing staff
• Skilled in using Microsoft Word, Outlook, and Windows systems
• Excellent team collaboration and communication skills
• Familiarity with hospice care standards and documentation best practices
• CHPN certification preferred or willingness to obtain within one year
