A well-established nonprofit mental health services provider in New York City is seeking a mission-driven Licensed Practical Nurse (LPN) to join their Assertive Community Treatment (ACT) team. This field-based opportunity focuses on providing innovative, community-based care to adults with serious mental illness and co-occurring substance use disorders who are involved in the criminal justice system.
The LPN will play a vital role in delivering compassionate, client-centered support that promotes recovery, wellness, and health integration. This is a highly collaborative role in a multidisciplinary team that includes behavioral health clinicians, medical staff, and social service professionals.
Title: Licensed Practical Nurse (LPN) – Assertive Community Treatment (ACT) Team
Location: New York, NY (Field-based role, community-focused)
Salary: $60,000 annually
Schedule: Full-time | 80% Field Work
Key Responsibilities:
• Deliver community-based health services in line with ACT model and LPN scope of practice
• Conduct wellness checks, vital sign monitoring, and medication administration (including vaccines, PPDs, and phlebotomy)
• Collaborate with the ACT RN to manage medication inventory and distribution
• Support clients in accessing primary care, medical treatment, and housing resources
• Monitor and report medication effects, adherence, and health concerns to clinical team
• Maintain up-to-date documentation in the Electronic Health Record (EHR)
• Ensure culturally responsive and trauma-informed care in all interactions
• Support coordination of care with ACT psychiatrists and other healthcare providers
Qualifications:
• Valid New York State LPN license (required)
• 3+ years of experience working with individuals experiencing mental health, substance use, homelessness, or justice involvement preferred
• Familiarity with community health systems and available support services
• Strong organizational and documentation skills
• Proficient in basic computer applications and electronic health records
• Spanish language fluency a strong plus
• Individuals with lived experience in mental health, substance use, or criminal justice involvement are strongly encouraged to apply
