Title: Nurse Practitioner – Primary Care
Location: Freehold, NJ & Whiting, NJ
Schedule: Full-Time (3 days in Freehold, 2 days in Whiting)
Pay: $70–$75 per hour
Setting: Senior Living / Geriatric Primary Care

What You’ll Be Doing
As a Primary Care Nurse Practitioner, you’ll play a vital role in providing high-quality healthcare services to senior residents across two vibrant communities.
Responsibilities

Conduct comprehensive assessments and physical examinations
Diagnose and manage acute and chronic health conditions
Develop, implement, and monitor individualized care plans
Prescribe medications and treatments as appropriate
Provide education and counseling to residents and families
Collaborate closely with physicians, nurses, and interdisciplinary team members
Maintain accurate and timely medical documentation
Participate in care team meetings and quality improvement initiatives
Ensure compliance with clinical, regulatory, and organizational standards

What We’re Looking For?

Master’s Degree in Nursing from an accredited program
Active New Jersey Nurse Practitioner license
FNP or AGPCNP certification preferred
Minimum 5 years of experience in primary care or geriatrics
Strong clinical assessment and decision-making skills
Experience caring for older adult populations
Excellent communication and relationship-building abilities
Team-oriented mindset with a compassionate, patient-centered approach
