Title: Nurse Practitioner – Primary Care
Location: Manchester Township, NJ
Schedule: Full-Time
Pay: $70–$75 per hour
Setting: Senior Living / Geriatric Primary Care

What You’ll Be Doing

Perform comprehensive health assessments and physical examinations
Diagnose and manage acute and chronic medical conditions
Create, implement, and monitor individualized care plans
Prescribe medications and treatment plans as appropriate
Educate residents and their families on health and wellness strategies
Collaborate with physicians, nurses, and other healthcare professionals
Maintain accurate and timely medical documentation
Participate in care team meetings and quality improvement initiatives
Ensure adherence to organizational policies and regulatory standards
Promote preventative care, wellness, and healthy aging

What We’re Looking For

Master’s Degree in Nursing from an accredited program
Active Nurse Practitioner license in the State of New Jersey
Family Nurse Practitioner (FNP) or Adult-Gerontology Primary Care Nurse Practitioner (AGPCNP) certification preferred
Minimum of 5 years of experience in primary care or geriatric medicine
Strong clinical assessment, diagnostic, and treatment skills
Knowledge and experience in caring for older adult populations
Excellent communication and interpersonal skills
Ability to thrive in a collaborative team environment
Compassionate, patient-focused approach to care
