Title: Registered Nurse – Community-Based Homeless Services
Location: New York City, NY (Field-based)
Salary + Benefits: $100,000 to $115,000 comprehensive health coverage, generous PTO, flexible hours, and the opportunity to be part of a high-impact statewide initiative.
Responsibilities of Registered Nurse – Community-Based Homeless Services:
Conduct assessments of participants’ physical, cognitive, and psychosocial health needs in field settings.
Engage participants in non-traditional settings (transit system, shelters, hospitals) and assist with transition planning.
Develop, implement, and evaluate care plans and interventions based on evidence-based practices.
Reassess participants regularly, especially when changes in condition or needs are observed.
Educate participants on health maintenance, disease prevention, diet, physical activity, and medication use.
Link participants to appropriate medical, specialty, and social service providers in the community.
Provide physical and psychological support in-home and community settings.
Collaborate closely with licensed clinicians, care managers, and peer specialists to coordinate holistic care.
Advocate for participants’ health needs across acute and non-acute healthcare systems.
Record medical information, vital signs, symptoms, and changes in condition accurately and timely.
Participate in team meetings, provide nursing insights into treatment planning, and engage in risk management.
Promote awareness of available community resources and work to reduce health disparities.
Support with medication reconciliation and monitoring for side effects.
Perform other nursing duties as needed in the field.
Requirements of Registered Nurse – Community-Based Homeless Services:
Current license and registration to practice as a Registered Nurse in New York State.
Minimum of 1 year of clinical nursing experience post-licensure, preferably working with individuals with behavioral health needs.
Strong communication, interpersonal, and organizational skills.
Willingness to work in a field-based role with flexibility in scheduling.
Passion for serving underserved populations and improving community health outcomes.
